Video Description

Remote patient management (RPM) improves care in a variety of ways. It provides clinicians with the ability to monitor patients between visits — as opposed to once every few weeks or months. RPM also promotes patient adherence to optimal health guidelines by providing daily feedback and reminders, as well as a more comfortable care environment: the patient’s home. RPM also encourages the proactive adjustments of care plans, as opposed to reactive changes, giving clinicians the chance to understand what’s going on with their patients “before it becomes a crisis, and before it becomes a higher cost of care,” in the words of the Care Innovations® experts.
